Accommodation
We provide a range of accommodation support services to people with disabilities.
Support is designed around the needs and requirements for each individual and can include individual or shared accommodation arrangements. In some instances, support arrangements are collaborative with families or personal networks sharing in the provision of support.
In all instances, ongoing involvement of families and friends is encouraged and supported. The maintenance of family contact is a high priority wherever possible.
Support arrangements range from drop in support to more intensive 24 hour support.
Short–term, intensive support is also available to eligible people from across the Mid North Coast area through the Mid North Coast Outreach Service.
All services are based around an Individual Service Plan which is developed with service users and families. This plan identifies the priorities for support, the aspirations of the service user, and shapes the support provided by CHSS. A report is provided twice per year detailing what CHSS has done toward the plan to ensure we “keep our end of the bargain”.
